The Home Sweet Home program with Buffalo Occupational Therapy is designed to promote Western New York Independent Living. It is for those looking for ideas to increase livability in their home throughout aging, those being discharged from the hospital with different levels of function, those with memory impairments, or those diagnosed with a degenerative brain disease and want to protect their decision to remain in their home.
What is Western New York Independent Living Consultation through the Home Sweet Home Program?
There is a common misconception that surviving a stroke, severe brain injury, living with mild cognitive impairment, a diagnosis of Alzheimer’s Disease, or any other neurological condition automatically warrants relocation to an assisted living facility, nursing home, or independent living apartment complex. While this change is welcomed by some, you may be feeling like you are being ‘forced’ out of your home! That’s where Home Sweet Home comes in! Don’t make this decision alone, let us help you! Our team of neurological and Aging in aging-in-place specialists are equipped with the knowledge necessary to help you decide whether or not you need to relocate, or if you can remain in your home with adaptations, training, or additional support. How does Home Sweet Home Work?
1. If you feel like you or your loved one need to live in an Assisted Living Facility, Nursing Home, or Independent Living Apartment community, contact us first.
2. Schedule a home assessment by a Certified Aging in Place Specialist (CAPS) where they will walk around the inside and outside of your home, recording details, taking photos, and gathering information to provide comprehensive feedback. If you also have concerns regarding your memory and movement abilities, there is an option for an occupational therapy practitioner to evaluate your function in a holistic manner (both out-of-pocket costs, no insurance accepted).
3. Buffalo Occupational Therapy will provide Independent Living suggestions and guidance on next steps.
