- Medicare/Medicaid Card
- Power of Attorney (if applicable)
- Letter of incapacitation (if applicable)
- Any discharge or medical paperwork applicable to your visit that you would like reviewed
Payment (if applicable)
If you have a copay for occupational therapy, please be prepared to pay this copay at the time of your visit.
